Intended Use
MedSafe Follow-Up is designed to support:
- Post-discharge medication follow-up workflows
- Medication education reinforcement
- Patient-reported barrier identification
- Follow-up prioritization activities
- Transitions-of-care communication initiatives
- Quality-improvement and operational reporting programs
- Healthcare organization pilot deployments
The platform is intended to support existing healthcare workflows and is not a substitute for clinical judgment, medical assessment, diagnosis, treatment, or emergency services.
By using MedSafe Follow-Up, you agree to use this platform only for its intended educational and follow-up purposes.
Healthcare Organization Participation
MedSafe Follow-Up may be deployed by hospitals, health systems, pharmacy departments, nursing teams, care-management programs, transitions-of-care initiatives, and quality-improvement departments.
Participating organizations remain responsible for clinical decision-making, patient care activities, and operational workflow oversight.
No Provider-Patient Relationship
Use of this platform does not independently establish a provider-patient relationship. MedSafe Follow-Up is a communication and education tool — it does not provide diagnosis, prescribe treatment, or replace professional medical judgment.

Medication Responsibility

Platform Availability
Services, workflows, reporting features, and educational content may evolve as part of ongoing quality-improvement efforts, pilot activities, maintenance, or platform development.
MedSafe Follow-Up does not guarantee uninterrupted availability, message delivery, system performance, or feature continuity.
User Conduct
Users agree not to:
- Misuse the platform for purposes other than intended
- Attempt unauthorized access to systems or data
- Submit false or misleading information
- Use the platform on behalf of others without appropriate authorization
Limitation of Platform
MedSafe Follow-Up supports communication, education, follow-up prioritization, and quality-improvement activities but does not guarantee:
- Improved clinical outcomes
- Reduced hospital readmissions
- Increased medication adherence
