Privacy Policy
How MedSafe Follow-Up collects, uses, and protects information to support post-discharge medication follow-up, transitions-of-care workflows, and quality-improvement activities.
Last Updated: September 22, 2026
Current Pilot Configuration
The current MedSafe Follow-Up pilot is designed around privacy-minimized, QR-code-based patient access:
- Patients open a private, single-episode QR link — no patient app or account is required
- No electronic health record (EHR) integration is required for the initial configuration
- A privacy-minimized configuration, designed to operate without protected health information (PHI) stored in MedSafe Follow-Up
How privacy and regulatory requirements apply to a given deployment depends on the participating organization's configuration, its patient population, and applicable law.
Information We Collect
MedSafe Follow-Up may collect limited information necessary to support post-discharge follow-up workflows, educational engagement, barrier identification, recovery-concern reporting, operational reporting, and quality-improvement activities.
- Medication education participation data
- Medication comprehension-assessment responses
- Patient-reported barriers to medication adherence and recovery
- Patient-reported recovery concerns
- Follow-up workflow activity
- Optional patient-reported support-experience response
- Operational and quality-improvement reporting data
Optional support-experience response: after completing a medication safety review, patients may be asked an optional question about whether access to MedSafe Follow-Up made them feel more supported by their hospital after discharge. Response choices are Yes, Somewhat, No, or Prefer not to answer. The question is optional, no free-text response is collected, and the response is used for program evaluation and quality-improvement reporting. It is not patient satisfaction, HCAHPS, a validated patient-experience measure, or a clinical outcome.
How Information Is Used
Information collected may be used to:
- Deliver medication-specific educational content
- Support post-discharge medication follow-up workflows
- Identify patient-reported barriers to medication adherence
- Facilitate review of follow-up opportunities by authorized healthcare personnel
- Support transitions-of-care communication activities
- Generate operational reporting and quality-improvement metrics
- Evaluate workflow utilization and patient engagement trends
- Evaluate optional patient-reported support experience and program engagement
- Improve platform functionality and educational content
Text Messaging in the Current Pilot
SMS/text messaging is not used in the current MedSafe pilot configuration. Patients access their medication safety review through a private, secure QR-code link, and a mobile phone number is not collected as part of the current pilot.
If an organization enables an approved messaging configuration in the future, additional communication data (such as a mobile phone number) and consent requirements may apply.
Quality Improvement and Pilot Activities
MedSafe Follow-Up may be used by healthcare organizations as part of quality-improvement initiatives, transitions-of-care programs, medication education projects, pilot deployments, or operational workflow evaluations.
Participation in these activities does not change patient privacy rights or alter how information is protected under applicable policies and procedures.
No guarantees are made regarding clinical outcomes, medication adherence, readmission reduction, or healthcare utilization.
Data Protection
We implement reasonable safeguards to protect your information, including:
- Encrypted data transmission where applicable
- Restricted administrative access controls
- Role-based workflow visibility
- Reasonable technical and operational safeguards
No electronic system can guarantee absolute security. MedSafe Follow-Up implements reasonable administrative, technical, and operational safeguards but does not warrant uninterrupted security, platform availability, or immunity from unauthorized access attempts.
Third-Party Services
MedSafe Follow-Up may use trusted third-party services to operate the platform:
- Secure cloud infrastructure providers
- Analytics and quality improvement services
- Application hosting providers
Information is not sold to third parties. Information may be shared only with authorized healthcare personnel and service providers necessary to operate, maintain, secure, and improve the platform. If an approved messaging configuration is enabled for a participating organization in the future, SMS delivery providers may be used for that configuration.
Your Rights
You may at any time:
- Contact support regarding privacy concerns
- Request discontinuation of applicable follow-up participation where appropriate
If a future SMS-enabled program applies to you, you may also opt out of text messages by replying STOP. That right is conditional on participation in an SMS-enabled program configuration.
